Hi and welcome to my online Web forms download page. On this page you will find downloads to common forms used on the Web. The forms are FREE! If you know a bit of PHP/HTML you can also customize them. Help standardize the industry with a form.

Before Your Download
In order for the form to work, you have to make sure your server can run PHP files. If you don't know, you can check with your ISP. I write code for both PHP and ASP/ASP.NET. If I get enough requests for ASP versions of the forms I can make a few of the more populars ones available. And finally, some of these Web forms are pretty ugly but they work where it counts. I've provided them as-is for now. Check back for updates or subscribe to my JT Web Forms Download list.

The above forms are starting points. You can use them as-is but if you are a designer then you can add your own customization. If you are not a designer and you want some pretty forms made then let me know. Uhm, I do that stuff. Ya'rly... There are a bunch of things I want to add such verification, database support, clean up the formatting and other useful things. If you need a form made please let me know and I will put it in my list to do. If I get enough requests for it I will make it and post the download.
